Hawthorne Chiropractic (Pro Atlas Trust trading as Hawthorne Chiropractic) recognises the importance of protecting the privacy rights of the individual in relation to personal information. This document is our privacy policy and explains how we collect, manage and disclose your personal information. We respect your rights under the Privacy Act and we comply with all of the Privacy act’s requirements in respect to collection, management and disclosure of your personal information.
Section A – Your Information
Hawthorne Chiropractic collects and keeps personal information given by you to us, as well as other information that may be forwarded by other health providers.
Section B – Information We Collect
All personal information is kept electronically and in paper form in our office.
All personal information is retained on a secure server and is protected from unauthorized access through the use of fire-walls, secure passwords, user logins or other security procedures.
Section C – Purposes for which we Collect, Hold, Use and Disclose Personal Information
The purpose for the collection of information is for the provision of chiropractic services to you.
We may at times use marketing to alert you to newsletters, reminders and promotions, If at any time you do not wish to receive further marketing information, you have the option to ask us not to send it to you. See details in the Contact Us section below.
As a standard practice, we do not give or share any personal information with third parties, unless they are acting as an intermediary. This would include any third party organizations involved in the operation, hosting and management of our information technology infrastructure (including our website), for example:
Section D – Accessing and Correcting Personal Health Information
As a patient you may access any personal information about yourself by contacting Dr Nicholas Armitage. Hawthorne Chiropractic will provide all material requested on receipt of a written request from you.
Section E – Complaints about a Breach of Privacy
If you believe we have failed to comply with the privacy Act or this Privacy Policy in relation to our handling your personal information, then you must first lodge your complaint with us.
We will treat your complaint confidentially. We will review your compliant and contact you within a reasonable time.
If we do not resolve you complaint, you make then take your compliant directly to the office of the Australian Information Commissioner (for more information, please see www.oaic.gov.au).
Section F – Personal Information Outside Australia
Your personal information will only be disclosed to overseas recipients, is specifically requested by you.
Section G – Disclosure of Personal Information to Overseas Recipients
Before we ever disclose personal information to overseas recipients (eg. A chiropractor overseas) we will seek your written consent.
